Texas Department of State Health Services is asking families to take part in a survey to find out more about services for children with special health care needs in Texas.

The Texas Department of State Health Services (DSHS) Maternal and Child Health Section is conducting a statewide Needs Assessment for the Title V Maternal and Child Health Block Grant. DSHS is asking families to take part in a survey to find out more about services for children with special health care needs in Texas. The information they share will give us an idea of what needs to be done to better serve children and youth with special health care needs.

The survey is open to parents/caregivers of CSHCN and young adults with special health care needs over the age of 18 in Texas. There is a parent/caregiver survey and a young adult survey in English and Spanish. Links to all four surveys are below. A flyer about the survey is attached. Please email MCHEpi@dshs.texas.gov to request paper surveys.

The Title V Needs Assessment also includes a community survey

DSHS would like to hear from Texas residents about community and family health needs and experiences across Texas. The survey takes about 15-20 minutes to complete, and you must be at least 18 years of age to participate.
