We are excited to announce that the Veteran Directed Care (VDC) program is now available in the Dallas-Fort Worth metroplex! This program is designed to help Veterans of all ages stay healthy and live independently at home, rather than moving to a nursing facility.
What is the Veteran Directed Care Program?
The VDC program, a partnership between the Department of Health and Human Services (HHS) and the Veterans Health Administration (VHA), aims to provide long-term services and support (LTSS) that are person-centered and participant-directed. This means Veterans can choose the care and services that best fit their needs, enabling them to live at home and stay engaged in their community.
How Does It Work?
Eligible Veterans in the VDC program receive an approved budget based on need, that they manage with oversight and support from a REACH care coordinator. They can decide on the mix of goods and services they need and even hire and supervise their own workers. This approach ensures that Veterans have control over their care and can tailor it to their specific needs.
Our Role in Care Coordination
REACH plays a crucial role in providing care coordination through the VDC program. We offer the following support to Veterans, their families, and caregivers:
- Facilitated Assessment and Care Planning: Our Care Coordinators work closely with Veterans to understand their needs and develop a personalized care plan.
- Fiscal Management Services: We help Veterans manage their approved budgets effectively, ensuring they can make the most of their resources.
- Ongoing Support: Our team provides continuous guidance and support to Veterans, helping them navigate their care options and make informed decisions.
